Are you accepting new patients?
Generally, yes. Availability varies by provider — some have openings, some have a waitlist. Tell us your insurance and what you're looking for and we'll tell you who's available.
How do I become a patient?
Submit an appointment request or call. Your request goes to a provider, who decides whether to accept you based on their availability, clinical fit, and whether they participate in your plan. Once a provider accepts you, they're your treating provider.
Can I choose my provider?
Yes. Request someone specific, or ask to be matched with whoever has availability.
FPC holds contracts with HMSA (all plans), UHA, HMAA, MDX, UHC, Ohana Care, Aloha Care, Humana, Cigna, Magellan, Tricare, VA, Medicare, Medicaid, and Kaiser. Participation varies by provider, because insurers credential clinicians individually rather than covering a whole practice. Tell us your plan and we'll confirm who's available to you before you book.
Most plans don't require one for behavioral health, but some do. Check with your plan, or ask us and we'll verify when we check your benefits.
See Fees and Insurance. Rates vary by provider, and your provider's current rates are confirmed with you before your first appointment.
It depends on the provider — some see children and adolescents, others adults only. Each provider's page lists the ages they see.
