This requires an ongoing treatment relationship. Disability and work-capacity opinions rest on how a condition actually presents over time — across multiple appointments, treatment changes, and periods of better and worse functioning. That cannot be established in a single visit.
As a rule, your provider will need to have treated you for several months before completing disability paperwork. If you are coming specifically to obtain documentation, please understand this before you schedule. An initial evaluation will not produce a completed form, and your provider may ultimately reach conclusions that don't support the claim.
This isn't a formality. A disability opinion is a clinical judgment your provider is putting their name and license behind, and it has to be one they can actually support.
